Holistic Care Coordination is a private Occupational Therapy service, providing 1:1 clinical support to family caregivers of elderly relatives and those living with dementia.

We assist families to navigate the health care system to achieve positive client outcomes, through assessment, advocacy, education, empowerment and referrals.

Our highly experienced clinicians work collaboratively with carers to optimise their loved one’s ability to participate in meaningful activities and improve overall quality of life.

Our work has a strong focus on the wellbeing of carers. When carers receive adequate support, feel empowered and have the resources they need, people receiving their care directly benefit.
